Year-End Reporting/"Check Out" in Annual Conference Statistics
The official Check Out deadline is January 22, 2025. Please check with your district office in case this date is different for your church. Below are instructions on logging in, as well as best practices. Please review these prior to logging in and filling out your Check Out forms.If you have questions, please contact your district administrator.

Check Out Forms Other Languages
For those churches whose primary language is either Korean or Spanish, check out forms in those languages are below. Churches may use these forms to arrive at their numbers, but the data itself must be filled in online and the only language available on the online forms is English.
